SMART Receives NY Health Foundation Grant

SMART is proud to announce that we are a recipient of the NY Health Foundation’s Healthy Food, Healthy Lives: Supporting a More Equitable Food System grant. With support from this grant, we have launched SMART Food for Life, a pilot project that will directly address the health of SMART women clients by linking those with Type 2 Diabetes (or who are pre-diabetic) to: a) nutritional counseling, b) the development of a medically tailored, culturally appropriate food plan, and c) the provision of prescribed food. Previous SMART OUTREACH PROJECTS!

SMART is extremely proud to have been part of the Vaccine Equity Partner Engagement Project (VEPE)! This initiative, led by the NYC Department of Health & Mental Hygiene and the Fund for Public Health in New York City, was designed to increase vaccine equity in communities across NYC that experienced low vaccination rates. SMART was one of a diverse group of community and faith-based organizations selected to offer factual information and engage their communities in conversations about the COVID vaccine. From August 2021 through January 2023, we mobilized small groups of our SMART women and youth to serve as outreach ambassadors and initiate discussions with community members, as well as disseminate targeted messaging we created. We were excited to apply our experience and compassion to the common goal of a safe and healthy future. Check out the VEPE webpage to review our successes!

SMART was also a partner organization of the New York City Department of Health and Mental Hygiene’s Monkeypox Virus Awareness & Prevention Partnership (MAPP). We were tasked with deploying proactive outreach and engagement in communities most at risk for Monkeypox Virus (MPV ) infection. The goals of the these activities were to help stem the outbreak and address inequities in health care access and outcomes through 1) raising awareness about the virus and local prevention, testing, and care options that are affirming and non-stigmatizing; and 2) connecting community members to services including vaccination appointments through navigation.
